What's the dumbest thing you've ever seen in a data center?

Data centers are great places to work, and hopefully most are built and managed well.  But sometimes things go wrong!  Maybe you've used a new supplier, or its the fault of a new hire, or maybe its just bad luck.  

So , what's the dumbest thing you've seen in a data center?

Photo/Video evidence would be nice :-) 

One Rule! No specific mention of the data center, nor the supplier, nor the name of the staff in question! :-)

Glass rack doors.
Recently: 
A cheap switch connected with ports 2&3 looped. 
Reflection and network crippled
Duct tape over ports to keep the line rate that was sold to them - no joke.
Water sprinklers
AMEN!
water or glycol cooled HVAC/CRAC units without redundant exterior ventilation or back-up chiller/condenser systems.
24 or more "legacy" 1U height servers stuffed into a single cabinet with no regard for heat-load or power consumption issues, with the accompanying excuse, "We don't want to waste any valuable space"!
discovering the reason a server lost network connectivity was due to someone re-using an ethernet patch cable with a broke-off retention clip on the RJ connector, and the connector slid-out of the jack on the server just enough to lose connection.
A stack of 4 servers, 2 DAS devices and a switch on top of a single 1U rackmounted server. They were short on rails or other rack mounting equipment so stacking was the easiest way forward.
